How to submit your sample / Cómo enviar tu muestra

🇺🇸 INSTRUCTIONS (ENGLISH) 🇺🇸

DATES OFFICE WILL BE CLOSED FOR SAMPLES: Currently OPEN

Please follow the steps below to submit your post-vasectomy sample:

1. Avoid sending your sample on the dates noted above ⬆️ (it will not be processed if it arrives during those dates)
   • * Consider it may take 5-7 days for the sample to arrive.
2. Collect your sample following the instructions in the video below.
   1. 1. Ejaculate in the previously provided medicine 💊 bottle
      2. Transfer the semen with the provided pipette (dropper) into the small vial
      3. Place the vial in the biohazard bag ☣️
3. Fill out the label on the biohazard bag.
4. Place the biohazard bag with the sample in the provided sample box.
5. Place the sample box inside the provided bubble mailer and drop it in your mailbox or at an U.S. Post Office. 📦
   • Please note that postage has been prepaid already (only USPS)
   • Write down the tracking number so you can confirm when the sample arrives at our office.
   • Your result will be available no later 48 hours of receiving your sample (they will be available in the Patient Portal HERE, so make sure you are registered)

‼️ Patients who got their procedure before July 1st, 2025 will have to pay the postage as prepaid envelopes where not provided before that date ‼️

** Remember **

1. 1. 1. If your sample is received before 12 weeks and 20 ejaculations, and needs to be repeated, an invoice will be sent to the Patient Portal for you to pay for a new sample kit that we will mail you.
      2. If your sample is received after 12 weeks and 20 ejaculations, and needs to be repeated, we will mail you a new sample kit at no cost.

FECHAS EN QUE LA OFICINA ESTARÁ CERRADA PARA MUESTRAS: Actualmente ABIERTA

Sigue los pasos a continuación para enviar su muestra post-vasectomía:

1. Evita las fechas de arriba ⬆️ al enviar tu muestra (no será revisada si llega en esas fechas)
   • * Ten en cuenta que la muestra puede tardar 5-7 days en llegar.
2. Recoge tu muestra siguiendo las instrucciones del video a continuación.
   1. 1. Eyacula en la botella de medicamento 💊 previamente provista
      2. Transfiere el semen con la pipeta (gotero) al pequeño vial
      3. Coloca el vial en la bolsa de riesgo biológico ☣️
3. Completa la etiqueta de la bolsa de riesgo biológico.
4. Coloca la bolsa con el vial en la caja de muestra.
5. Deposita la caja de la muestra en tu buzón de correo, o llévala a una oficina de correos (USPS) 📦
   • Ten en cuenta que el costo de envío ya está prepagado (sólo para USPS).
   • Escribe el número de seguimiento para confirmar cuándo llega la muestra a nuestra oficina.
6. Tus resultados estarán disponibles dentro de 48 horas luego de recibir tu muestra (se enviarán al Patient Portal AQUÍ, asegúrate de registrarte)

‼️ Pacientes quienes tuvieron su procedimiento antes de Julio 1, 2025 tendrán que pagar el envío ya que los sobre prepagados no se entregaban antes de esa fecha ‼️

** Recuerda **

1. 1. 1. Si tu muestra es recibida antes de 12 semanas y 20 eyaculaciones, y necesita ser repetida, te enviaremos al Patient Portal un recibo de pago que debes cancelar por un nuevo kit de muestra que te mandaremos por correo.
      2. Si tu muestra es recibida después después de 12 semanas y 20 eyaculaciones, y necesita ser repetida, te enviaremos por correo un nuevo kit de muestra, sin costo.
